No Shows and Late Cancellations:
New patients or patients with multiple late cancellations (less than 24 hours) may not be rescheduled and may be charged a fee. You will get a confirmation notification with the date and time of your appointment by email, text, and phone call. Please confirm that you can attend your appointment. Failing to appear for an appointment may result in a $50 to $100 charge. This must be paid before you are scheduled again. No-shows or late cancellations for initial appointments may result in not being rescheduled.
For new patients, please create an account in the Patient Portal. Make sure to thoroughly fill in your Past Medical History, Medications you are currently taking, and all other sections.
Please bring recent lab work or doctor notes.
Please bring your driver's license and insurance card.
Please have records faxed to us at (713) 600-9002.
Prescription refills must be called to the pharmacy. Please allow up to 72 hours for prescription refills.
Please allow 5 to 7 days for all referrals.
If you receive a bill from an outside lab (QUEST, Lab Corp), please call them first to inquire (1-866-846-4021), so we can better assist.
Billing departments make errors. If you receive a large bill from our office, do not panic. Please contact your insurance first, then inform us of the situation, so that we may correct any error.
